Consultation

My goal is to relieve your pain

Initial visit, 60 minutes, in-person or virtual consultation for those who have never seen Dr. Soh: $600. (If you have previously been seen by Dr. Soh within the last year, you may schedule a follow up visit after providing a copy of previous office visit notes)

  • At Soh Headache Center, understanding your goals and who you are is important to establishing an optimal and individualized diagnosis and treatment plan. 
  • Before your first visit, you will fill out a headache questionnaire that we’ll review together during your visit. You will need to submit this headache questionnaire prior to scheduling the first visit. 
  • At the initial visit to our center, expect a 45-60 minute session consisting of many questions. It is okay if you don’t know all the answers. We are trying to get to know you, and every detail about your clinical history helps. 
  • We will perform a physical examination if in-person. 
  • We will discuss your condition in detail, answer your questions, and set treatment expectations. 
  • You will leave with a custom approach to getting you better. 
  • Please fax your records, labs, and imaging prior to appointment. If you have any imaging such as an MRI brain, CT head, or CTA of the head and neck, please bring CD of images to first appointment. Your imaging center should be able to provide you a CD. 
  • Expect homework such as recording a headache diary/journal.

Follow-up visits

Professionally trained and certified

Follow up, 20-30 min (in-person, telemedicine): $300

  • At the follow-up visit, we go over how you’re doing, the results of any tests, and your homework. 
  • We will ask questions about lifestyle such as diet, sleep, exercise, mood, as well as potential medication side-effects. 
  • We will do a short physical exam if in-person. 
  • We continue to further customize your health treatment plan. 
  • If you are feeling better, follow-up visits may serve as a “check-in” periodically to make sure all is well, renew prescriptions, and provide any life updates. 
  • If your headaches have been stable and you are doing well, you may be able to follow up with your primary care provider for continued medical management.

Nerve blocks

Your rehabilitation starts with a proactive approach

Nerve blocks: $250

  • Using a very small needle, nerves around the head are coated with a small amount of numbing agent that can reduce the severity of a headache or completely eliminate it.

Trigger point injections

Both nerve blocks and trigger point injections can be performed during pregnancy and are relatively safe procedures when performed by a trained headache specialist.

Trigger point injections: $200

  • Using a very small needle, taught areas of muscle are injected with a numbing agent to help alleviate pain and muscle tension.

Botox

Chronic migraine prevention

Botox injection fee: $600 (30 minute procedure not including drug cost)

  • Botox given in standard injection sites around the head and neck can help reduce the frequency and intensity of chronic migraine.
  • We will authorize Botox medication through your insurance and use a specialty pharmacy to supply the medication. 
  • The Botox savings program can help with out-of-pocket costs when using insurance for medication coverage. 


Botox injection fee and Botox drug ($1300): $1900

  • If you do not plan to use health insurance for drug coverage, then this would be the cost of procedure and medication. 
  • The charge for this service is made at time of appointment scheduling.

Ketorolac injections and infusion therapy

Reducing pain

Ketorolac intramuscular injection: $50

  • Can be given during an office visit or combined with nerve blocks, trigger point injections, and other medications to treat migraine headache acutely. 


Outpatient infusion therapy:

  • For existing patients, outpatient infusion therapy can be ordered.
  • A three day infusion plan would require a follow-up visit prior to infusion and after infusion.



*Services paid by credit/debit card will be charged processing fee of 5%.

Other

Protecting you

Prior authorizations for medications: $50 per medication per year

  • Obtaining prior authorizations for medications can take significant amounts of time and this fee supports administrative efforts.


FMLA and accommodation forms: $100 per letter or form

  • We aim to keep our patients with headache disorders working and in their jobs. We should know you well before being entrusted to fill out this paperwork for you. Therefore, filling out this paperwork is at the discretion of your doctor. Under certain circumstances, we may not be able to fill out these forms.
